You're right, the writers do need to work on the interaction between Castiel and the Winchesters. You can't go from "omg are you okay??" to "Cas? Who's he? That guy back there?"
Dean is bemoaning his opportunity to bask in the presence of the real (fictional) Dr. Sexy. Poor Dean. (And now I have sad visionsn of Dean Mary Sue-ing himself into Dr. Sexy, MD...That's scary.)
